County Boys Player Registration and Parental Consent Form.

As part of our Safeguarding and H&S policies this form must be completed by the parent/guardian in conjunction with the player and submitted to give consent for the the player detailed below to attend any County coaching sessions, representing the County and playing in any County run events. This form must be received prior to attendance. Please note if any of the information changes you must submit an updated form.

Covid 19 Declaration - you accept sole responsibility to ensure all current Government, Public Health or any relevant Golf governing bodies advice & guidance is followed in respect of you or your child when attending any L&RGU County Boys Training sessions, events or matches.
