County Boys Player Registration and Parental Consent Form.

As part of our Safeguarding and H&S policies this form must be completed by the parent/guardian in conjunction with the player and submitted to give consent for the the player detailed below to attend any County coaching sessions, representing the County and playing in any County run events. This form must be received prior to attendance. Please note if any of the information changes you must submit an updated form.

Covid 19 Declaration - you accept sole responsibility to ensure all current Government, Public Health or any relevant Golf governing bodies advice & guidance is followed in respect of you or your child when attending any L&RGU County Boys Training sessions, events or matches. If as a result of any of this guidance and advice you need to withdraw your child from this any of the County Training sessions, Events or Matches then you must do so with immediate effect and notify the Director of Junior Golf by email to lrgu.juniorgolf@gmail.com.

Data Protection Policy all the information supplied above will be kept in strictest confidence on a secure database and will only be used by Officers and Officials of the Leicestershire and Rutland Golf Union for the purposes of organising competitions, coaching, matches or other activities for the Junior Section (including results published in the press) and will not be disclosed to any third party.
