County Event Parental Consent Form.

As part of our Safeguarding and H&S policies this form must be completed for all Junior entrants by the parent/guardian in conjunction with the player and submitted to the County prior to playing in any County run events. Please note if any of the information changes you must submit an updated form.

Data Protection Policy all the information supplied above will be kept in strictest confidence on a secure database and will only be used by Officers and Officials of the Leicestershire and Rutland Mens Golf Union and/or the Ladies CGA for the purposes of organising competitions (including results published in the press) and will not be disclosed to any third party other than if required for Covid 19 NHS Track and Trace purposes.
